Making a claim means requesting payment for your medical expenses where you were required to pay for the service upfront. A completed UHIP claim form provides Sun Life all the necessary information from your medical visit, such as the location, the type of service and the cost of treatment. Always bring a claim form with you when visiting a health care provider.

When to submit a UHIP Claim Form
When visiting a preferred health care provider, they may send the request for payment directly to Sun Life. You will still be required to fill out a claim form and give it to the health care provider.
However, not all health care providers will send the request for payment directly to Sun Life. In this case, you will have to pay out of pocket for your medical expenses. You will then need to submit a claim form to Sun Life in order to request reimbursement. If you are submitting the form yourself, always make sure to attach the original receipts or bills and have your form signed by the treating health care provider. Always keep a photocopy of your claim form and all receipts when you send the originals to Sun Life.
You and the health care provider can always contact Sun Life at 1-866-500-UHIP (8447) to confirm what expenses are covered under UHIP. This is always a good idea if you are not sure.

Claim Processing & Status
Payment will be processed and a cheque mailed to you or the health care provider within seven calendar days from the date the UHIP claim form is received at Sun Life. If you prefer to receive reimbursement by direct deposit into your Canadian bank account, you will need to provide your banking information to Sun Life at mysunlife.ca. All claims must be submitted to Sun Life no more than 12 months after the date you received the health or medical service; otherwise, you may not be reimbursed.
